ABSTRACT:
Diabetes mellitus is a chronic metabolic disorder characterized by impaired glucose regulation, necessitating lifelong pharmacological intervention. Conventional insulin administration and oral antidiabetic therapies often suffer from limitations such as poor patient compliance, variable absorption, and risk of hypoglycemia. In recent years, microneedle (MN)-based transdermal systems have emerged as a promising frontier in diabetes management due to their minimally invasive nature, ability to bypass first-pass metabolism, and potential for painless self-administration. Microneedles fabricated from polymers, metals, or biodegradable materials enable the controlled and sustained delivery of insulin and other antidiabetic agents directly into the dermal microcirculation. Furthermore, integration with glucose-responsive polymers and continuous glucose monitoring devices offers the potential for closed-loop, smart delivery platforms tailored to individual patient needs. This review explores the current landscape of MN-based drug delivery systems, with emphasis on fabrication techniques, material considerations, and mechanisms of drug release. Recent advances in dissolving, hydrogel-forming, and stimuli-responsive microneedles are highlighted, alongside preclinical and clinical evaluations. Critical challenges such as scalability, sterility, patient acceptability, and regulatory aspects are also discussed. The article concludes with future prospects, particularly the integration of microneedles with wearable biosensors and personalized medicine, positioning them as a transformative strategy in diabetes care.
